To provide you with the best care, it is important for your provider to understand your medical and mental health history, living situation, relationship status, and employment history.
Be ready to list any medications you take including doses and any allergies to medications to the best of your knowledge. Please also provide names and contact information of any other medical providers that you have visited, so that we may better coordinate your care. Save time and fill out your forms before your appointment. Simply print out the packet, fill them out and bring them in to your first appointment.
